Where can I get papers and stuff to transfer custody without getting a lawyer? -

There is a 16 year old girl thats mother is incapable of taking care of her. She had a son with my cousin which is already under the care of my Great Aunt. The girl is the boys half sister. My great aunt want to get custody of him but she doesn t want to have to pay all the money for a lawyer. Where can i get the paper work and stuff for her to do that?
